Interim Caregiver Services

Interim Caregiver Services (ICS) provide temporary support and assistance to individuals before long-term care arrangements are in place.

The ICS programme is managed by the Agency for Integrated Care (AIC); It aims to facilitate hospital discharge for patients so they can be at home as they wait for their long-term care plans to come into effect.

How is ICS and Home Personal Care (HPC) Different?

ICS defers from normal HPC services as they are intended to be a form of transitional care until long-term care arrangements come into place after being discharged.

This means that ICS can go for up to 12 hours per session for a 2 week period and patients must be referred to the program by their respective hospitals. Meanwhile, HPC services are implemented as required for a shorter duration per session but occur over a longer period of time.

How to apply for Interim Caregiver Service?

Patients are typically referred to the program by their respective hospitals, which handle the application process on their behalf.

Who Can Perform Interim Caregiver Services?

Interim Caregiver Services can be provided by trained professionals such as Nurses and Care Aides before the arrival of your foreign domestic worker or before other caregiving arrangements are in place.

Are There Subsidies for Interim Caregiver Services?

Subsidies for Interim Care Services is provided to eligible individuals.

You are eligible for ICS if: 

  • You are referred by AIC, or discharge nurses and medical social workers from restructured hospitals/community hospitals.
  • You are deemed medically stable for discharge but still require post-discharge assistance at home.
  • You possess documentation confirming arrangements for ongoing care, awaiting implementation (e.g., nursing home placement, centre-based services, home care service, or arrival of a foreign domestic worker).
  • Your referral is made within five (5) calendar days following your hospital discharge

The standard duration subsidised by the Authority for ICS is 12 shifts over two weeks. Shifts may be from 8am to 8pm, or 10pm to 8am. If any extension of service is required beyond the standard duration, an appeal can be made to AIC.

What Services are Offered During These Sessions?

During Interim Care Sessions, Jaga-Me offers a range of services including:

Caregiving and Respite Care

Trained caregivers offer assistance with ADLs and provide respite for family caregivers

  • Elder sitting
  • Help with Activities of Daily Living  (ADLs)
  • Bathing, Dressing, Oral or Tube Feeding, Toileting, Mobility, Transferring
  • Meal reminders
  • Medication reminders and supervision
  • Mind-stimulating activities
  • Physical maintenance exercises


Medical Escort Services

Medical Escort

  • Prepare and accompany patients for medical appointments,
  • Assist them during the medical appointment (eg. toileting), and
  • Send patients home after medical appointment.
Light Housekeeping

Light Housekeeping*

  • Laundry
    Machine washing, drying, folding, hanging up (subject to laundry quantity and service hours)
  • Changing bed linen
  • Wiping down kitchen counters after meals
  • Wiping down bathroom countertops and shower/tub after bathing
  • Maintenance-level cleaning of the toilet
  • If a toileting accident occurs, cleaning the immediate area as needed
  • Taking trash to the collection spot
  • Sweeping and mopping the patient’s living space
  • Tidying the patient’s living space

*The amount of light housekeeping that can be completed is subjected to patient’s living space and service hours. Priority of service rendered goes to ensuring the care and safety of the patient.

Nursing/Doctor Procedure

When necessary, our Care Aide can seek the advice of our Nurses/Doctors for clinical issues raised/observed during caregiving appointment (if a clinical procedure is required, additional charges for nursing/doctor procedure applies).
